    Please print out, complete, then bring to the first appointment in our office the Patient Demographics Form and the Acknowledgement of Receipt of Notice of Privacy Practices (last page of the Notice of Privacy Practices) to facilitate your visit. When your demographic information (address, telephone number, insurance, etc.) changes, please bring in an updated Patient Demographics Form to your next visit.
